Visitor forms
If you're planning a visit to Blue Iris Farm, we require all visitors to fill out three important forms: a Photo & Video Release, a General Waiver, and an Emergency Contact Form.
The Photo & Video Release allows us to take photos and videos of our visitors while on the farm, which we may use for promotional or educational purposes. By signing this form, you give us permission to use your likeness in any such material.
The General Waiver is a standard liability waiver that releases Blue Iris Farm from any responsibility for any injury, loss, or damage that may occur during your visit. We ask that all visitors sign this form before entering the farm premises.
The Emergency Contact Form asks for the name and contact information of someone we can reach in case of an emergency during your visit. This is a precautionary measure to ensure that we can act quickly and effectively in case of an unexpected situation.

Animal surrender forms
At Blue Iris Farm, we understand that sometimes, difficult circumstances may arise where you are no longer able to care for your beloved animal companion. That's why we offer an animal surrender program to help find loving homes for animals in need. Our surrender process is designed to be as smooth and stress-free as possible for both you and your animal friend.
To begin the surrender process, simply fill out our Surrender Application Form. This form provides us with important details about your animal, such as their health history, behavior, and any special needs they may have. Our team of compassionate experts will review your application and contact you to discuss the next steps.
Once your application is approved, we'll ask you to sign our Surrender Contract, which outlines the terms & conditions of the surrender agreement. This contract ensures that your animal will receive the best possible care and find a loving forever home. We want to assure you that your animal's welfare is our top priority, and we are committed to working together to find the best possible outcome for them.

Donor forms

There are several ways to support our organization through donations. If you have items or goods that you would like to donate, please fill out our In-kind Donation Form. This form helps us keep track of donations and ensures that we receive the items we need to care for our animals.
For those interested in corporate partnerships, we have a Corporate Sponsorship Form. This form provides businesses with the opportunity to support our organization and receive recognition for their contributions. Our team will work with you to develop a customized sponsorship package that aligns with your company's values and goals if none of our options fit your needs.
For all donations, we want to ensure that we are transparent and accountable for your contributions, we have a Donor Agreement Statement below. This statement outlines our commitment to responsible stewardship of donations and our pledge to use donations for the intended purposes.
